I was inspecting a vessel with a near-perfect safety record.
☑️Paperwork: immaculate.
☑️Drills: conducted and logged.
☑️KPIs: green across the board.
Then I watched a crew member walk under a suspended load without a second thought.
No one said anything. Including me, in that first moment.
Because it was so normalized, so embedded in how that crew operated that it barely registered.
The metrics said 'safe.'
The culture said something completely different.
That's the Wakashio problem. And it's more common than anyone wants to admit.
