International Nurses Day

On International Nurses Day, I want to acknowledge nurses working in aged care. The work you do is complex, demanding, and deeply human, yet it is still too often taken for granted.

Through my own professional work and personal experience supporting family members in care, I’ve seen the level of responsibility and judgement aged care nurses manage daily. You notice the small changes, ask the hard questions, advocate when it’s uncomfortable, and stay present day after day. That depth of knowledge and care makes a real difference.

Thank you for the skill, compassion, and professionalism you bring to this work. It matters more than people often realise. 💜

Support at Home is about prevention and risk rarely announces itself.

Support at Home is Australia’s home aged care program, designed to help people stay safe, independent, and living at home for longer.

Prevention isn’t just about avoiding falls or hospital admissions.
It’s about recognising the pathway that leads to them.

Most serious incidents are preceded by small, often subtle changes such as shifts in appetite, balance, confidence, engagement, or cognition. On their own, these changes may seem minor. Together, they can signal emerging risk.

This is where prevention under Support at Home truly sits:
not at the point of crisis, but well before it.

For families, this means trusting your instincts when something feels different and raising concerns early.
For providers, it means noticing early change, sharing observations across the care team, and responding before risk escalates.

If we wait for an obvious decline — a fall, a hospital stay, a crisis — we’ve already missed the opportunity to act early.

The goal is to prevent avoidable decline — or at least minimise its impact — before that decline leads to harm, hospitalisation, and loss of independence.

This focus on early recognition, shared responsibility, and timely adjustment of supports is central to the work I do at Judge Consulting Works, supporting organisations and leaders to strengthen preventive practice under Support at Home and embed it into everyday decision‑making.

Falls and slips don’t always happen when a support worker is in the home.

Sometimes they happen between visits, and families or clients aren’t always sure whether they should tell the care provider or how to do that.

When falls or near misses aren’t shared early, the care team may not know that someone is becoming unsteady. This can mean support changes, equipment, or safety strategies are put in place later than they should be.

Early signs matter.
A slip, a near fall, or more frequent falls can be a sign that extra help or small changes at home could prevent an injury.

This isn’t about blame.
It’s about making sure changes are shared early so people can stay safe at home.

If you’re comfortable, you’re welcome to share your experience in the comments — many families and clients deal with similar situations.

And if you’d rather talk privately or want help knowing what to raise with your care provider, you’re welcome to message me.

One thing I often see in aged care…

Good intentions paired with overwhelm.
Providers and families are navigating:
• Regulatory change
• Clinical risk
• Quality standards
• Complex care decisions

What’s often missing isn’t commitment — it’s clear, grounded guidance.
Experienced consulting isn’t about “fixing” people or services.

It’s about walking beside you, helping you see the next right step.
Clarity reduces stress. Confidence improves outcomes.
