So many investigations barely scratch the surface resulting in a superficial root cause analysis. The subsequent actions do little to reduce systemic risks in an organisation. In this article, I discuss a regulator's investigation into a fatality and how the findings reveal little in the way of learning or substantive reduction of risk. I hope this review leads you to question your organisation's approach to investigations.
