My name is Erica Stephenson and I am a pharmacist with accreditation to conduct Home Medicine Reviews (HMRs) in the Northern Sunshine Coast region of Queensland.

A Home Medicine Review involves me visiting with you in your home for 1-2 hours. You can have family, a friend or carer present if you like. We go through all of your medications and medication-related problems together - you can tell me anything that you think I need to know. After the review, I prepare a confidential report for your GP, who may choose to make changes that could improve the use of medicines for you.

Home Medicines Reviews are FREE with a doctor's referral.

You can ask your doctor for a Home Medicines Review. Your doctor can send me a completed referral request directly via email to [email protected].
