When it comes to negotiating contracts with medical centres, there’s usually not a huge amount of wiggle room.

Most practices already have existing doctors on set terms so to keep things fair, new GPs generally need to fit within that structure. Otherwise, you risk creating inequality in the team, which quickly turns a recruitment problem into a retention problem.

That said, there can be room for flexibility if you're willing to take on things that others might not. After-hours work, weekends, supervising international doctors or registrars… these are areas where practices are more likely to offer better service fees or extra incentives.

Let’s talk about long-term contracts because they can make doctors nervous.

When I’m chatting with GPs early in the process, one of the biggest concerns is committing to a multi-year contract. The usual thought is: “What if it doesn’t work out?”

Totally fair question.

But what I always point to is the exit clause. If a practice offers a 2, 3, or even 4-year contract but includes a 30-day opt-out, that changes everything.

It’s not really about the length, it’s about flexibility. Personally, I’d be just as comfortable signing a 4-year contract as a 1-year one, as long as I knew I had that option to leave if things didn’t feel right.

One thing I’ve learnt? The most successful placements happen when both the practice and the doctor are upfront from day one about their expectations, availability, and what they’re looking for.

When there’s alignment early, you avoid surprises later. No one wants to get halfway through the recruitment process only to realise it’s not the right fit.

Clear, early conversations save time and lead to stronger, longer-lasting working relationships.

Timing also matters. Practices want to know when you're realistically available; short, medium, or long term, so they can plan accordingly. They’re also considering how your special interests and clinical strengths complement their current team, and whether you’re likely to build a loyal patient base over time.
