Tip control is assumed in advanced endoscopy and almost never measured.
HAM-2, out now in Endoscopy International Open: a cooked-ham model with four standardised 3D-printed shapes, traced with snare tip soft coagulation, scored by blinded raters.
13 endoscopists, 104 STSC procedures at Ghent University Hospital.
At baseline the model separated experience levels — subjective tip control 95.3 (expert) vs 59.7 (fellows) vs 56.3 (trainees), p < 0.001; speed 40.6 vs 23.6 vs 19.7 mm/min.
After two unsupervised, structured self-practice sessions: tip control +12.3 points (95% CI 6.9-17.7), speed +5.5 mm/min, correct hit frequency +6.1 hits/min. Accuracy did not move (+1.3%, p = 0.085) — the ceiling was already above 90%.
The practical point: the endoscopists with the lowest baseline speed and accuracy improved the most. Deliberate self-practice, no supervisor, no simulator suite, one piece of ham.
