Schools are charging students a quarter million to half a million dollars in tuition. Without any business training, paying that back in a reasonable timeframe becomes extremely difficult.

My first clinic was in the red for nearly a year. Twelve months of losing money. Easy to say now, but living through it felt like twenty years.

Your education does not stop at dental school. The business side of practice ownership is not required by your provincial dental board, but it will pay off in a way that nothing else in your career will.

You do not know what overhead is normal, what supply costs are too high, or what actually constitutes a good deal. Without an established baseline, every single vendor quote looks standard, and you can end up bleeding profit before you even open your doors.

You took the leap into ownership. That means working on the business, not just in it.

In our clinic, every Wednesday is a meeting day. Admin, hygiene, assistants all meet separately. Then on the final Wednesday of the month, a liaison from each department sits in the same room for cross-department communication.

It sounds simple because it is. But without it, issues get buried in the chaos of the day. Things do not get communicated. Problems compound quietly until they become expensive.

Structured meetings keep the whole operation aligned. Schedule them and protect them.

Most dental practices are running their supply costs anywhere from 7% to 9%. If you can bring that down to the 4% to 6% range, the day-to-day difference might seem small on paper.

But you have to look at the real numbers. When you extrapolate that tiny percentage drop over $1.5 million or $2 million in revenue, it completely changes your bottom line. That small shift equals tens of thousands of dollars going straight back into your pocket at the end of the year.

There is a cost to change. Avoiding it might feel comfortable now, but it catches up.

Think about it this way: there was a point where refusing to get a driver's license or use the internet went from a personal preference to a serious disadvantage. Dentistry has those same tipping points.

When enough clinics have adopted new technology, you stop being traditional and start being the clinic stuck in the old times. Patients notice. Then they go down the street.

Do you bite the bullet now and absorb the friction early? Or do you bury your head in the sand and hope it works out?

The moment you open a text or email in the inbox, it is yours. You own it from start to finish. No passing it around. No hoping someone else picks it up.

The best way to explain it to your team: walk up to someone, ask them to lick a spoon, then hand it to someone else. Watch their reaction. "I don't want it." Exactly.

When a shared inbox has no clear ownership, accountability disappears. Inquiries sit unanswered for weeks and patients walk away frustrated.

New patients are the lifeblood of any clinic. But the ones slipping out the back door quietly are costing you more than you realize.

Patient churn represents lost revenue from people who already know you, already trust you, and just fell off somewhere along the way.

Tracking it turns you from reactive to proactive. And that is the difference between a clinic that grows and one that just stays busy.

Not mentioning a treatment option (no matter the cost) isn't neutral. It's a decision you made for your patient.

And if they find out later — from a second opinion, a specialist, a Google search — that there was another path you never offered? You didn't just lose their trust, you confirmed their worst suspicion about dentists, and we already get enough bad press.
If you offer Treatment Option X, the worst they say is "no thank you", and you still sleep well at night.
If you DON'T offer Treatment Option X, then you run the risk of a college complaint and less than optimal sleep.
Informed consent isn't just a form they sign, it's a conversation you owe them.
Every option. Every time. Even the ones you'd talk them out of.
