OPERATIONAL TRUTHS #06 - Control During Failure

Operational leadership is easy when systems are stable.

Pressure reveals the real operating model.

Major incidents expose:

• decision bottlenecks
• fragmented leadership
• governance weakness
• dependency risks
• accountability confusion

This is where organisations discover whether operational control actually exists.

Because during failure conditions, PowerPoint governance disappears very quickly.

The uncomfortable truth?

Many organisations only discover the weakness of their operating model after operational pressure has already escalated.

OPERATIONAL TRUTHS #05 - Fragile Architecture

Architectural fragility rarely announces itself openly.

It appears indirectly.

Slow delivery.
Change hesitation.
Escalation dependency.
Release anxiety.
Excessive approval layers.

Eventually organisations become afraid of their own environments.

Because nobody fully trusts what impact change might trigger.

This is where operational maturity begins collapsing.

Teams slow down.
Governance becomes defensive.
Innovation weakens.
Technical debt accelerates.

The uncomfortable truth?

Many organisations are not slow because they lack capability.

They are slow because the environment underneath has become operationally fragile.

OPERATIONAL TRUTHS #04 - Stability is a Feature

Enterprise IT beyond the slide deck.

Many organisations are obsessed with appearing innovative.

New tooling.
New frameworks.
New transformation branding.
Constant reinvention.

Meanwhile their operational foundations remain unstable underneath.

Major incidents continue.
Escalations increase.
Delivery confidence weakens.
Teams become reactive.

Because stability is not seen as exciting.

But mature organisations understand something important:

Operational stability is not the opposite of innovation.

It is the prerequisite for it.

The strongest enterprise environments are usually not the loudest.

They are the ones quietly engineering resilience, simplification, accountability, and operational consistency while everyone else chases transformation theatre.

The uncomfortable question:

Is your organisation engineering operational maturity — or simply marketing transformation?

OPERATIONAL TRUTHS #03 - Governance Without Control

Enterprise IT beyond the slide deck.

Many enterprise environments are overloaded with governance activity.

* Steering groups.
* Review boards.
* Escalation forums.
* Risk committees.
* Reporting layers.

Yet when operational pressure arrives, decision-making still collapses.

Because governance activity is not the same as operational control.

Real governance changes organisational behaviour.

It creates clarity.
Ownership.
Authority.
Decision velocity.

But many governance models simply create administrative theatre around unresolved operational dysfunction.

The uncomfortable truth?

If leadership still cannot make fast, confident operational decisions during failure conditions, the governance model is not operationally mature.

It is performative.

OPERATIONAL TRUTHS #02 - Transformation Theatre

Many organisations are not transforming.

They are relabelling existing dysfunction with modern terminology.

New platforms.
New tooling.
New delivery structures.

Yet underneath:

* ownership remains fragmented
* operational instability survives
* governance remains reactive
* accountability remains unclear

The technology changes.
The organisational behaviour does not.

This is why many transformation programmes generate activity without producing operational maturity.

The uncomfortable truth?

Transformation that never changes the underlying operating model is usually theatre.

OPERATIONAL TRUTHS #01 - Visibility is Not Control

Enterprise IT beyond the slide deck.

Most CIOs don’t actually have operational control.

They have reporting.

* Dashboards.
* Service reviews.
* Governance packs.
* Metrics everywhere.

Yet the same incidents repeat.
The same escalations return.
The same operational instability survives every transformation programme.

Because visibility is not control.

Many organisations are highly informed about problems they still cannot operationally prevent.

A dashboard cannot stabilise a broken operating model.

And governance means very little when accountability is fragmented across suppliers, teams, and leadership structures.

The uncomfortable truth?

If operational stability still depends on manual intervention, escalation chains, and tribal knowledge, the organisation is not operating under control.

It is operating under observation.

Multiple suppliers doesn’t mean resilience.

It usually means:

• Fragmented ownership
• Misaligned SLAs and OLAs
• Confusion during incidents
• Finger pointing instead of resolution

And when a major incident hits?

No single view.
No clear leader.
No control.

Just noise.

If your service relies on multiple suppliers…
your operating model matters more than ever.

Because they’re not managing your service.
You are.

The biggest IT costs don’t show up in your budget.

They show up in:

• Lost user productivity
• Slow processes
• Repeat incidents
• Delayed delivery

10 minutes of downtime × 5,000 users = a problem.
Even if Finance never sees it.

This is where most CIOs are exposed.

Because they can report spend…
but can’t always show cost of inefficiency.

And that’s where the real money is being lost.

ITIL doesn’t fail.
Agile doesn’t fail.
DevOps doesn’t fail.

Ex*****on fails.

We see organisations implement frameworks “by the book”
…then wonder why nothing improves.

Because frameworks aren’t solutions.
They’re tools.

And tools in the wrong hands:

* Add process without clarity
* Add governance without control
* Add overhead without outcomes

The question isn’t:
“Are we using ITIL?”

It’s:
“Is ITIL actually working for us?”

“It works.”

That’s the most dangerous sentence in IT.

Because underneath it usually sits:

• Manual workarounds
• Hidden failure points
• Poor user experience
• Silent productivity loss

It’s not broken enough to fix.
But it’s not right either.
So it stays.

And over time?

That “it works” system becomes the one that fails when it matters most.
