Job Description – Support Worker (Lone Working) Main Duties (not in order of priority):
To provide specialist support to care leavers by providing supported living and appropriate accommodation, so that they can stay safe and achieve their potential and succeed economically within their community.
Transition Care provides high impact measurable, support with the following key features
Professional assessment and interventions supported through individualised support plans
Safe and appropriate accommodation within the Young Person’s community
Initial high-level support moving to tapered support as independence and life skills are achieved and evidenced.
Move on assistance with sourcing accommodation education and employment opportunities as part of the program.
Staff on call 24hrs a day for support and advice.
Our company provides person centered, realistic, achievable community-based support for young adults who are vulnerable, thus promoting the persons independence continually in line with a personalisation agenda.
The service is able to provide professional assessments and agree individualised support and protection plans with Statutory agencies, with tapering support packages in and outside of the home, Inc. healthy eating, staying healthy and fit, building self-esteem, community awareness, managing budgets, practical skills and robust routines ensuring post program sustainability.
Develop effective working relationships with the other employees within the Service.
Support an open, positive and inclusive working culture.
Ensure Service User rights are protected.
Encourage a model of self-care and Service User rehabilitation.
Record relevant activities in Care Plans.
Seek opportunities for personal and professional growth.
Promote a positive image for residency and employment within the Service.
Support the implementation of the Service's policies and procedures.
Support and maintain a culture of performance and excellence.
We require worker who are 21year old and over due the age of our service user group.
Although not essential as training will be given, preferred candidates will have NVQ/QCF Level 2 in Health and Social Care.
Part-time hours: 16-37 per week
Job Types: Full-time, Part-time, Temporary, Permanent
Salary: £9.00-£10.50 per hour
