These are the three questions I am being asked most often by Registered Managers and Nominated Individuals right now.

Not theoretical questions.
Not academic ones.
Real concerns coming from people running services under pressure.

First, managers are trying to understand what CQC actually expects them to evidence under the Single Assessment Framework. Many have policies, audits and trackers but are unsure whether these genuinely demonstrate quality across the Quality Statements or simply look good on paper.

Second, there is growing anxiety around personal accountability. Managers are increasingly aware that when governance fails, scrutiny does not stop at the organisation. They want to know how to evidence professional challenge, decision-making and escalation so they can demonstrate they acted appropriately if something goes wrong.

Third, there is the daily reality of firefighting. Staffing pressures, operational demands and constant problem-solving mean governance can easily become reactive. Managers are asking how to stay compliant without drowning in paperwork or creating systems that collapse under pressure.

These questions tell us something important.

Care managers are not asking for more documents.
They are asking for clarity, prioritisation and defensible governance.
