This was a quick response to troubleshooting a local server. The OS drive was accidentally ejected by a staff who had little knowledge about servers in his attempt to manually shutdown the server in the absence of an IT officer. His action completely crushed the disk.

To resolve this fatal incident as quickly as possible, I tried several methods to repair the disk using all available tools but to no avail. Fortunately I had cloned the OS drive some time back so I just swapped the drives and everything was back online.
