🔍 POLICY vs PRACTICE REALITY CHECK
Policy says: Fall-risk assessment.
Patient condition changes: Reassessment missing.
🚨 Where is the gap?
A fall-risk assessment is not simply a score to be completed in the patient file. When a patient's condition changes, the hospital must identify the change, reassess the risk, update preventive measures and document the response according to its defined process.
💡Quality Tip:
Don't audit only “Was the fall-risk score documented?”
Ask:
“Did the assessment respond to the patient's changing risk?”
🎯 Today's Quality Challenge:
Review 5 patients whose clinical condition changed and trace:
Change → Reassessment → Updated Risk → Preventive Action → Monitoring

💬 Would you consider this a documentation gap, an implementation gap, or a patient-safety gap? Why?
