This is an annual report containing the verified itemized listing of establishments’ labor component, specifying the names of their workers and employees below the managerial level, including learners, apprentices and workers with disabilities who were hired under the terms prescribed in the employment contracts, and their corresponding salaries and wages.

Magbayad ng SSS, PHILHEALTH at PAGIBIG contributions ng inyong mga empleyado. CRIMINAL CASE at may KULONG ang parusa sa mga employer na susuway sa batas na ito at nagkakaltas sa kanyang mga manggagawa.

Isang negosyante ang timbog matapos siya sampahan ng reklamo ng Social Security System (SSS) na hindi siya nagbabayad ng SSS contribution para sa mga empleyado ng kanyang bakeshop sa Cagayan de Oro Miyerkoles.
