➖ Monthly Salary: PHP 31,705.00 PLUS 20% PREMIUM PHP 6,341.00

➖ Nature of Appointment: CONTRACT OF SERVICE
➖ Place of Assignment: MANAGEMENT SUPPORT DIVISION-MEDICAL ASSISTANCE FOR INDIGENT AND FINANCIALLY INCAPACITATED PATIENT PROGRAM SECTION
➖ DEADLINE: JULY 26, 2026
Interested and qualified applicants should signify their interest in writing. Submit the following documents

1. Intent letter indicating the Position Title, Place of Assignment;
2. Fully accomplished Personal Data Sheet (PDS) with Work Experience Sheet and recent passport-sized or unfiltered digital picture (CS Form No. 212, Revised 2025); digitally signed or electronically signed and duly notarized;
3. Hard copy or electronic copy of Performance rating in the last rating period (if applicable);
4. Hard copy or electronic copy of proof of eligibility/rating/license (e.g. Screenshot of CSC Eligibility from csevs.csc.gov.ph or PRC Board Rating and Identification Card with screenshots of verification of Rating and License through online.prc.gov.ph);
5. Hard copy or electronic copy of Transcript of Records and Diploma;
6. Certificate of Trainings/Seminars attended; and
7. Photocopy of Certificates of Employment.
