Beliefs like our patients will only accept what insurance pays for. Or I do not want to diagnose treatment I cannot schedule for months. These beliefs feel like facts. But they are not facts. They are walls and on the other side of those walls are possibilities that most hygienists never get to see.

Katherine shares that the worst case scenario of challenging a belief is that you challenge it and decide you still believe the same thing. There is no downside to asking the question. But there is a very real cost to never asking it.

Your clinical standard of care should never be written by an insurance company.

And yet it happens every single day in dental practices across the country. A patient needs treatment. Insurance denies it. Someone on the team says we cannot do that because insurance will not cover it. And just like that the recommendation stops.

Here is the distinction that every dental team needs to understand. Benefit criteria and diagnostic criteria are not the same thing.

What an insurance company will pay for and what is clinically appropriate for your patient are two completely different standards. When we treat them like they are the same we hand our clinical authority over to an insurance plan.

A real example. A patient was treatment planned for a crown due to bruxism. The plan only covered crowns for traumatic injury or significant decay. Insurance denied it. But the clinical need did not change. The tooth still needed that crown.

The same thing happens in periodontal care all the time. Radiographic calculus may be required for an insurance benefit for scaling and root planing. But radiographic calculus is not required for you to diagnose periodontal disease. Those are two different standards.

Document the clinical case. Present what the patient needs. Let the insurance conversation be separate from the clinical recommendation.

You set the standard of care. Not the insurance company.
