I keep coming back to this because I think we blame clinicians for something they often don’t have much control over.

We tell people to collaborate.

But the therapist is in one practice. The RD is in another. The prescriber is somewhere else. They have different systems, different schedules, different ways of documenting, and usually no built-in time to actually coordinate.

Sometimes there just isn’t structure there to support communication. I think we need to get a lot more curious about what collaboration actually requires.

The goal of collaboration is to give your expertise more context.

A dietitian’s understanding of a nutrition pattern can change when they understand the psychological context behind it.

A therapist’s understanding of a behavior can change when they understand the nutritional or physiological context.

A prescriber’s understanding of a symptom can change when they understand what’s happening outside the exam room.

Different lenses don’t compete. They expand each other. Your clinical lens is powerful. And it grows with collaboration.

I think there’s a big difference between knowing other disciplines exist and actually knowing when their lens would change your clinical picture.

That second one is a clinical skill.

And it’s one I wish we talked about a lot more in professional training.

A referral shouldn’t be the first time another clinician enters the clinical picture. Sometimes the most important part happens before the referral: Recognizing that the question you’re asking might not be the only question worth asking.
