Most leaders think great teams avoid conflict.

10 years after I left a company, a podcast guest described the exact same problems I experienced there.

Same patterns. Same ceiling. Same reasons good people kept leaving.
A decade had passed. Leadership had changed. And the system was still winning.

That's the thing about broken systems — they don't fix themselves just because time passes or new people show up. They keep producing the same results until someone finally decides to understand the system and change it.

W. Edwards Deming spent his career trying to teach leaders this. Most still haven't learned it.
