🕯️Florence Would Have Noticed

Florence once said:
"The very first requirement in a hospital is that it should do the sick no harm."

More than 150 years later, that principle remains unchanged.

What made Florence extraordinary wasn't just compassion.
👀 It was observation.
She understood that patients rarely go from stable to critical without warning.

There are clues:
📈 A changing respiratory rate.
😴 A subtle shift in mental status.
📉 Oxygen saturation slowly declining.
👩⚕️ A nurse's intuition.

Individually, they seem insignificant.
But together, they tell a story.

📊 Failure to recognize clinical deterioration remains a recurring factor in patient safety events and malpractice claims.

🔍 One of the most important questions in any review is:
When was the first opportunity to recognize something was changing?

Because the answer often explains everything that followed.

🕯️ Florence taught us that observation saves lives.
The records often tell us whether anyone was truly observing.

🔔 The Alarm Everyone Heard

The monitor alarmed.
Again.
Again.
Again.
Eventually, everyone became accustomed to hearing it.

That's the danger.
Healthcare calls it alarm fatigue.

Risk managers call it a predictable hazard.
⚖️ Attorneys call it a question:
"What happened after the alarm sounded?"

Research shows clinicians may be exposed to hundreds of alarms during a single shift.
When everything feels urgent, nothing feels urgent.

⚠️ The alarm wasn't missed because nobody heard it.
It was missed because everybody heard it.

Whether it's a surgery center recovery unit, skilled nursing facility, or hospital floor, the risk is the same.

Some moments remind you exactly why you do what you do.

On Friday, I had the incredible privilege of welcoming my beautiful new nephew into the world. 💙

As a nurse with a passion for patient safety and quality care, it's impossible to simply "turn it off." Even as a proud aunt, I found myself doing what healthcare professionals do without even realizing it: observing, assessing, watching communication, patient identification, hand hygiene, workflows, equipment, and the countless small details that work together to keep patients safe.

Not because I'm looking for something to be wrong.

Because I know just how incredibly fragile those first moments of life are, and I know that every detail matters.

Watching a brand-new life enter this world was a powerful reminder that every patient, whether they're minutes old or ninety years old, is someone's whole world. Behind every chart is a person, a family, and a story.

Congratulations to my sister and brother-in-law on your perfect little miracle. Welcome to the world, little man. You are already so deeply loved. 💙

👨👩👧 The Family Was Telling You
"Something isn't right."

Families say it every day.
Sometimes they're reassured.
Sometimes they're dismissed.
Sometimes, no one investigates further.
But families know what is normal for their loved one.

In many investigations, the earliest warning signs didn't come from a monitor.
They came from:
👩 A spouse.
👨 A son.
👧 A daughter.
Someone noticed a subtle change before anyone else.

Communication breakdowns continue to be one of the leading contributors to serious patient harm events.

⚠️ The issue often isn't that the warning wasn't given.
The issue is that no one acted on it.

Sometimes the biggest clue wasn't hidden in the chart.
🪑 It was sitting in the visitor chair.

The Copy-and-Paste Trap

"Patient resting comfortably."
Three words.
Repeated shift after shift.

Meanwhile:
📈 Vital signs are worsening.
😣 Pain is increasing.
👨👩👧 Family concerns are escalating.

Documentation should tell a story.
Too often, it tells the same story over and over.

As a Legal Nurse Consultant, one of the first things I look for is whether the documentation matches what was actually happening.

⚖️ Because when the chart says one thing and the evidence says another, people pay attention.
~Attorneys.
~Experts.
~Jurors.
📝Documentation issues are among the most common factors identified in malpractice claims and patient safety investigations.

The chart isn't just a record.
It's the story that remains when everyone else's memory fades.

Everyone focuses on the patient who fell.
Few people focus on the patient who almost fell.

🦽 The wheelchair that nearly rolled away.
🚶 The stumble caught by a staff member.
✋ The handrail grab that prevented a fall.
Because when no injury occurs, many organizations move on.

But patient safety professionals know better.
Every near miss is a free lesson.
Every close call reveals a weakness in the system.
📊 For every serious safety event, there are often dozens of precursor events and hundreds of near misses that occur first.

❓The question isn't:
"Did someone get hurt?"
The question is:
"What almost happened?"
Because today's near miss can become tomorrow's lawsuit.

🔍 That's true whether you're running a surgery center, assisted living community, skilled nursing facility, or hospital.
