When something goes wrong, our first instinct is usually to look for someone to blame. 👇
👤 The manager missed a step.
👤 The employee made a mistake.
📦 The vendor dropped the ball.
But after working with communities across the country, we’ve found that most operational chaos can be traced back to one of three things:
✔️ No process
👀 No visibility
📋 No accountability
If three employees perform the same task three different ways, that’s not a people problem.
That’s a systems problem.
Before asking who caused the issue, ask:
Does the process exist?
Can the team see the problem?
Does someone own the outcome?
Most chaos isn’t created by bad people.
It’s created by unclear systems. 🔄
