Care Navigation helps families understand their options. It provides guidance, education, and direction through the often-complicated healthcare and aging services systems.
Care Management builds on that foundation by helping coordinate care, communicate with providers, advocate for needs, and ensure that plans are put into action.
When these two approaches are combined, families receive both the knowledge they need to make informed decisions and the support and assistance they need to carry them out.
My goal is not simply to provide answers—it’s to help families understand their choices, feel confident in their decisions, and know they have someone walking alongside them through the process.
