You probably already have a ChatGPT tab open right now.

Maybe you use it to draft a social post, rewrite a form letter, or answer a question at 11pm you didn't want to Google. And it feels like progress. Like you're one of the "AI-forward" ones.

Here's what a new Goldman Sachs survey of 10,000 small businesses found: 76% of small businesses are now using AI. Only 14% have actually built it into how they operate.

That's the gap most practice owners never notice. Using a tool a few times a week isn't the same as having a system that runs without you. You can have AI open in five different tabs and still be the one writing every post, replying to every review, making every decision that keeps your practice moving. Nothing about your actual week changes.

There's a new way practices are losing patients, and most owners don't even know it's happening.

It's not a bad review. It's not outdated branding. It's something quieter: when someone asks AI who to see, some chiropractors just... don't come up.

I've coached a lot of chiropractors through hard seasons. I told Bill directly on this episode: I'm so proud of you. And I meant every word.

Three practices, all leaning on the same insurance payer. When that payer changed its model, referral volume disappeared and took everything down with it. Two bankruptcies followed.

That's the weight Bill carried for years. Today, more than half his practice runs on cash instead of insurance. Less staff. Less overhead. More time for his family. And enough confidence now to say he'd open another practice without hesitation.

He told me on this episode: "I wouldn't be scared to go open up another practice, because I know a cash-based practice can go anywhere."

AI doesn't listen. It reads. ⤵

That distinction, from DrEd Osburn on this episode of In The Blue, is the one most chiropractors are missing entirely.

Every podcast, every video, every post you make should have a transcript attached to it. Captions, SRT files, full transcripts on YouTube, all of it. Not for your audience. For AI.

Here's the part most doctors don't think about: iTunes and Spotify aren't just places people listen. They're search engines. When someone (or AI) searches for who to see for a specific condition, if you don't show up there, you don't get referred. Period.

The doctors getting recommended right now aren't necessarily the best ones. They're the ones AI can actually find and read enough about to trust.

You've built something you're proud of. You're also more exhausted than you know how to explain to anyone who's never run a practice.

I've spent more than 3,500 hours coaching practice owners, looking at patterns across over 2,000 practices. The exhaustion you're carrying right now, I've heard it described almost the exact same way more times than I can count.

I was standing there in Kenya, about as far from home as I'd ever been, and I felt how small my own plans really are.

I used to think if I just gripped tighter, worked longer, gave more of myself, I'd finally feel like enough. I never did. I just felt more hollow. More alone in a life that looked full from the outside.

I was never supposed to carry any of it alone. Not in my faith. Not in my business, either.

From the outside, everyone thought he had it figured out.

3 practices. Fast growth. One of them went from $0 to $200K a month in collections in under 6 months.

Then his primary insurance payer changed its model, and the referral volume that had been carrying all three practices disappeared almost overnight.

Two bankruptcies. Ten years spent digging back out.

Here's the part that stings the most: even at his peak, close to $2 million in revenue, he was only keeping about 5 cents of every dollar. Because the whole thing ran on one marketing engine and one insurance payer, not a system.
