The staffing conversation in pharmacy often focuses on hiring, but the structure of the work itself plays an equally important role.

In many pharmacies, responsibilities have evolved over time without a clear reset. Tasks are added, processes shift, and expectations expand, often without redefining roles or adjusting how work flows through the business.

This creates pressure at the team level. Not because people aren’t capable, but because the system they’re working within hasn’t been intentionally designed to support how the pharmacy operates today.

Clarity in roles, workflows, and decision-making allows the team to operate with consistency and focus. It creates a more stable environment for ex*****on and supports stronger performance over time.

Staffing is directly connected to how the business is structured.

Where in your pharmacy would greater clarity in roles or workflows make the biggest impact?

Technology adoption has become a priority across pharmacy but stability is what actually determines performance.

I see environments where new tools are introduced with good intent, but without a clear connection to how the business operates day to day. The result is a system that requires more coordination, more manual work, and more effort to maintain.

Technology should support consistency in how the pharmacy runs. It should reflect real workflows, allow information to move cleanly, and give the team clarity in how work gets done.

When that alignment is in place, ex*****on becomes more predictable. The team spends less time navigating systems and more time moving the business forward.

Stability creates the conditions for efficiency, scalability, and long-term growth.

Where is technology creating unnecessary complexity in your pharmacy today?

“We’ve always done it this way” tends to go unchallenged in busy pharmacy environments.

Over time, processes expand. Steps are added, systems evolve, and responsibilities shift. What once worked efficiently can become harder to manage without anyone intentionally redesigning it.

This is where profitability is often impacted—through time, workflow complexity, and resource allocation that no longer align with how the business operates today.

A structured review of operations creates visibility:
Where are delays occurring?
Which tasks require more effort than they should?
What has changed in the business that the process has not kept up with?

An annual review of workflows helps ensure the business is operating with intention and alignment.

What is one process in your pharmacy that would benefit from a closer look?

Most pharmacies say they want to grow. Fewer can clearly explain how their business actually runs today.

I see this pattern often: strong intent, good people, steady volume—and behind the scenes, inconsistent workflows, underutilized technology, and decisions made without clear visibility.

Operational clarity isn’t a nice-to-have. It’s what allows everything else to work.

Here’s what it looks like when it’s in place:

Workflows are consistent. The team knows what “right” looks like and executes it the same way every time.

Technology supports the business. Systems are aligned with operations, not layered on top of them.

The pharmacy has a defined role in its community. Services, positioning, and patient experience are intentional—not reactive.

These aren’t abstract ideas. They show up in margins, time, and team performance.

Where do you see the biggest gap in pharmacy operations right now—workflows, technology, or positioning?

Independent pharmacists are increasingly speaking before lawmakers about the operational realities of PBM reimbursement.

Through FAIR Rx Act advocacy efforts, pharmacy owners are sharing firsthand perspectives on spread pricing pressures, reimbursement unpredictability, and the impact on patient access in their communities.

These conversations matter. When legislators hear directly from frontline providers, policy discussions become grounded in real-world experience rather than abstract data.

Advocacy is not separate from business strategy. It is a component of protecting sustainable community-based care.

Federal PBM reform provisions are moving from discussion to action.

Newly enacted and proposed measures are targeting rebate transparency, spread pricing practices, and “any-willing-pharmacy” protections — all areas that directly influence independent pharmacy sustainability.

These reforms are designed to bring greater accountability to pharmacy benefit structures and improve clarity around how reimbursements are calculated and distributed. While implementation details will continue to evolve, the direction is clear: increased scrutiny and structural oversight at the federal level.

Independent pharmacy owners who understand these provisions can better anticipate operational impact, contract implications, and long-term reimbursement strategy.

Federal policy is shaping the reimbursement environment of tomorrow.

State-level PBM reform efforts are gaining momentum across the country, and for independent pharmacies, that shift carries real operational implications.

Legislators are increasingly focused on reimbursement transparency, fair payment models, and patient access protections. These discussions directly influence margin stability, cash flow predictability, and the long-term viability of community pharmacies.

Independent owners who understand the evolving policy environment — and monitor how reforms unfold in their state — are better positioned to make informed strategic decisions. Staying engaged in these conversations supports both business sustainability and patient access in the communities they serve.

Revenue growth is not the same as profitability.

Independent pharmacies are working harder than ever to increase volume (more prescriptions, more services, more activity). But more revenue does not automatically mean more stability.

Margin is what determines sustainability.

When reimbursement tightens and acquisition costs fluctuate, volume can create the illusion of strength while profitability quietly erodes. A pharmacy can be busy all day and still feel financial pressure at the end of the month.

The real question isn’t “How do we grow revenue?”
It’s “How do we protect margin while we grow?”

That means evaluating purchasing strategy, understanding contribution by service line, and building workflows that support efficiency, not just activity.

Growth without margin discipline creates fragility.
Growth with financial clarity creates control.

And long-term success in independent pharmacy requires control.
