แนวทางปฎิบัติตามนโยบาย Phuket Sandbox สำหรับผู้เดินทางจากประเทศยูเออีทั้งชาวไทยและชาวต่างชาติ
1. เงื่อนไขการเข้าร่วม
1.1 เป็นผู้ที่ได้รับวัคซีนครบถ้วนเเล้วไม่น้อยกว่า 14 วัน โดยต้องเป็นวัคซีนที่ได้รับการรับรองจากรัฐบาลไทย หรือได้รับการรับรองจากองค์การอนามัยโลก (WHO) ได้แก่ AstraZeneca Sinovac Johnson&Johnson Moderna Sinopharm Pfizer เเละ CovidShield ** เด็กอายุต่ำกว่า 18 ปี ที่ยังไม่ได้รับวัคซีนสามารถเดินทางมาพร้อมกับผู้ปกครองได้
1.2 เดินทางจากประเทศที่มีความเสี่ยงต่ำ/ ปานกลาง ซึ่งรวมถึงยูเออี และต้องพำนักอยู่ในยูเออีติดต่อกันเป็นเวลาอย่างน้อย 21 วันขึ้นไป
1.3 ต้องทำประกันสุขภาพที่ครอบคลุมค่าใช้จ่ายในการรักษาพยาบาล (รวมทั้งการรักษาที่เกี่ยวข้องกับโรคติดเชื้อโควิด-19) วงเงินไม่น้อยกว่า 100,000 ดอลลาร์สหรัฐ ซึ่งบริษัทประกันสามารถเป็นได้ทั้งบริษัท
ที่อยู่ในประเทศไทยหรือในประเทศอื่น ๆ
1.4 เเสดงเอกสารยืนยันการจองเเละชำระเงินโรงเเรมครบถ้วนตามมาตรฐานความปลอดภัยด้านสุขอนามัย (SHA+) โดยสามารถค้นหารายละเอียดได้ที่ https://www.facebook.com/Shaplusphuket/
1.5 ต้องพำนักในจังหวัดภูเก็ตไม่น้อยกว่า 14 วัน จึงจะสามารถเดินทางออกไปยังพื้นที่อื่น ๆ ในประเทศได้ กรณีจะพำนักในจังหวัดภูเก็ตน้อยกว่า 14 วัน เมื่อครบกำหนดเเล้วต้องเดินทางออกจากประเทศไทย
ตามที่กำหนดเท่านั้น
1.6 กรณีจะเปลี่ยนโรงเเรม ต้องพำนักอยู่ที่โรงเเรมเเรกเป็นเวลา 7 วัน จึงสามารถย้ายไปพักที่โรงเเรมอื่นในจังหวัดภูเก็ตที่อยู่ในเกณฑ์ของ SHA+ เช่นกันได้ (ย้ายได้ไม่เกิน 2 โรงเเรม)
1.7 ก่อนเดินทาง ต้องเเสดงผลการตรวจหาเชื้อโควิด-19 โดยวิธี RT-PCR ระยะเวลาไม่เกิน 72 ชั่วโมง
1.8 ระหว่างที่พำนักในจังหวัดภูเก็ต 14 วัน ต้องเข้ารับการตรวจหาเชื้อโควิด-19 จำนวน 3 ครั้ง (ครั้งที่ 1 เมื่อเดินทางถึงสนามบิน ครั้งที่ 2 ในวันที่ 6-7 ของการพำนักเเละครั้งที่ 3 ในวันที่ 12-13 ของการพำนัก) โดยต้องรับผิดชอบค่าใช้จ่ายครั้งละประมาณ 2,800-3,500 บาท ด้วยตนเอง เเละจองผ่านโรงเเรมที่พัก หรือหน่วยงานสาธารณสุขจังหวัดภูเก็ต
1.9 ลงทะเบียนขอรับใบรับรองการเข้าประเทศ (Certificate of Entry - COE)
1.10 ยินยอมให้ติดตั้งระบบติดตามตัวตามที่ทางราชการกำหนด ตลอดระยะเวลาที่พำนักในจังหวัดภูเก็ต
2. ขั้นตอนการลงทะเบียนเพื่อขอรับใบรับรองการเข้าประเทศ (Certificate of Entry - COE)
2.1 ลงทะเบียนทางเว็บไซต์ https://coethailand.mfa.go.th/ ล่วงหน้าเป็นเวลา 3-5 วันทำการ
2.2 ขั้นเเรกในระบบ เลือก "ใช่" ที่ประเภทการเดินทาง Sandbox Programme ในส่วน "ข้อมูลส่วนบุคคล"
2.3 กรอกข้อมูลส่วนบุคคล พร้อมเเนบหลักฐานการฉีดวัคซีน เพื่อให้เจ้าหน้าที่ตรวจสอบ
2.4 ขั้นที่สองในระบบ เมื่อผ่านการตรวจสอบเเละได้รับอนุมัติชั้นเเรกเเล้ว โปรดเเนบหลักฐานการจองโรงเเรมที่ได้การรับรองจาก SHA+ ตั๋วโดยสารเครื่องบิน เเละหลักฐานการจองการตรวจหาเชื้อโควิด-19 จำนวน 3 ครั้ง ในประเทศไทยที่ชำระเงินเเล้ว
2.5 รอผลการอนุมัติ เมื่อได้รับอนุมัติ พิมพ์เอกสารออกมาเพื่อใช้ประกอบการเดินทาง

Phuket Sandbox Travel Guidelines for Travellers from the UAE
1. Traveller Eligibility (both Thai and Non-Thai nationals):
1.1 Fully vaccinated against COVID-19 for no less than 14 days before departure with vaccines approved by the Thai government or WHO: AstraZeneca, Sinovac, Johnson&Johnson, Moderna, Sinopharm, Pfizer and CovidShield ** Children under the age of 18, traveling with parents, are eligible without proof of vaccination;
1.2 Travel from countries considered as low/ medium-risk areas including from the UAE.Those who are not residing in the UAE must have continuously stayed in the UAE for at least 21 days before departure to Phuket;
1.3 Hold a valid health insurance that covers medical expenses including treatments relating to COVID-19, with a minimum of 100,000 USD, throughout the period of stay in Thailand;
1.4 Hold a confirmed booking of stay at a hotel in Phuket approved only by SHA+ for at least 14 days. For more information: https://www.facebook.com/Shaplusphuket/;
1.5 Compulsary stay in Phuket for at least 14 days before tralveling to other provinces in Thailand. In case a traveller prefers to stay less than 14 days in Phuket, early departure from Phuket is possible ONLY for destinations outside Thailand;
1.6 Traveller can either stay for 14 days at the same hotel or 7 days at the first hotel and the rest 7 days at other hotels in Phuket (no more than 2 hotels);
1.7 Upon check-in at the airport, a traveler must present an RT-PCR negative test result issued no more than 72 hours;
1.8 Must take 3 COVID-19 PCR tests during the stay in Phuket (1st test on arrival at the airport, 2nd test on Day 6-7, 3rd test on Day 12-13). Fees of approximately 2,500 - 3,500 Thai Baht for each test are not included in the hotel packages, please contact the hotel for the exact price and reservation;
1.9 Hold the Certificate of Entry (COE) issued by the Royal Thai Embassy in Abu Dhabi and the Royal Thai Consulate-General in Dubai;
1.10 Must install and allow the use of tracking applications throughout the stay in Phuket
2. Registration for the Certificate of Entry (COE):
2.1 Register via https://coethailand.mfa.go.th/ 3-5 working days prior to the departure date;
2.2 Pre-approval: Choose "Yes" for the Sandbox Programme in "Travel Details" section;
2.3 Fill in personal information and attach proof of vaccination;
2.4 Final approval: Attach SHA+ confirmed hotel booking, flight tickets and booking of 3 COVID-19 PCR tests in Phuket;
2.5 Once the COE is approved, print out and carry with yourself throughout the journey
