At RTR our main focus as risk consultants is to use the common tool “hierarchy of controls” as our platform, and to concentrate on identifying & eliminating any high level risks you might not be aware of or items that may have slipped under the radar. These areas might be areas that you have slowly seen coming to light as your business has grown or they might be risks that have not yet been identified, with new developments come new risks. It may be that you are about to enter a whole new business venture. These are the items we are really concerned about and would like to help you with. Statistics have shown us that when a fatality occurs in the work place, or a catastrophic event has taken place & a root cause investigation will normally follow; the findings will generally indicate that there what was a missing link or pathway that was not identified within the initial mitigation controls. This is certainly the area that we are here to help you identify - " the real time Risk." RTR = the one causal factor that was overlooked or not identified during the initial risk & hazard assessments, that has the potential to kill or cause catastrophic consequence to health or our environment. Even with all the general safety tools implemented and mitigation planning in place like; toolbox meetings, Job Safety Analysis (JSA) Step Back 5x5 being used at the coal face of our work fronts, and in most cases high-level risk assessment (RA) tools like HAZOP and HAZID being conducted prior to the engagement, evidence shows that we are still have nasty incidents that can and have lead to a fatality or a non-desirable catastrophic events. This is where RTR comes into play;

By aiming higher and being more prepared, by providing the right representation with experience to your work site we can continually focus on RTR while at the same time educate your work force about how they all can assist in looking out for real time risk. Big milestone personally and professionally.
After years working across offshore construction, marine operations, diving, oil and gas, defence related projects and high consequence environments, I’ve officially completed my first book:

THE ILLUSION OF CONTROL
The Hidden Gap Between Governance and Operational Reality

This book is not theory from a classroom.
It is built from real world operational exposure, investigations, near misses, leadership observations and lessons learned across some of the highest risk industries in the world.

The core message is simple:
Many organisations believe they are in control because systems, procedures and compliance frameworks exist on paper.
But operational reality in the field often tells a very different story.

The book explores: • governance blind spots
• human factors and operational pressure
• why experienced people still make fatal errors
• the disconnect between executive assurance and frontline reality
• lessons from marine, offshore, construction and high risk environments
• how weak verification creates hidden exposure long before incidents occur.

This project has taken a huge amount of time outside long site hours and consulting work, so seeing it finally move to publication is something I’m proud of.

Reduced to an incident in minutes

That is the reality of high risk industries
If controls are not actively verified.

You are relying on assumption, not assurance

Multiple explosions were heard at the refinery, which provides 10 per cent of the nation’s fuel.

Regulators don’t prosecute paperwork.
They prosecute failure of control.

If you are a business owner or PCBU, this sits with you.

Not your safety advisor.
Not your supervisor.
You.

You can have every policy, SWMS and procedure in place and still be completely exposed.
Because documents do not control risk.
Verification does.
If you are not actively confirming that controls are in place, understood and working in real conditions, then you are not managing risk.

You are guessing.
And that guess gets tested the moment something goes wrong.

When regulators step in, they will not be interested in what you intended.

They will look at what actually happened.

-Were the controls there
-Were they followed
-Were they effective

If you cannot prove that, your system has failed.

And as the PCBU, that failure sits directly with you.

This is the gap most businesses ignore.
Not the system.

The absence of real verification.
That is where incidents come from.
That is where liability sits.

Understanding the law around high risk work is not optional.

If your controls are not properly implemented and an incident occurs, the regulator will not stop at your documents.

They will look at what was actually happening on site.

They will test:
• Were the risks properly identified
• Were controls suitable and effective
• Were they implemented and verified

If the answer is no, exposure sits with the business and its leaders.

This is where accountability becomes real.
High risk work demands more than paperwork.

It demands understanding, implementation, and ongoing verification.

You can have the best safety systems, policies and culture on paper.

So why are significant incidents still happening?

Because systems don’t fail on documents.
They fail in ex*****on.

In high risk work, exposure sits in the gap between what is written and what is actually happening on site.

I still see:
• Controls not understood
• Procedures not followed
• Risk assessments not aligned to the actual task
• Supervisors consumed in getting the job done, without space for verification not through complacency, but through pressure and deadlines.

When an incident occurs, it is not the document that gets tested first.

It is the reality of what was happening at the time.

If your controls are not being actively verified in the field, your system is not protecting you.

It is only giving you a false sense of assurance.

Strong systems matter.

But verification of implementation is where risk is actually managed.

Most organisations don’t have a systems problem they have a verification problem.

I want to be clear about something.

Real Time Risk is not a document seller.

Our core work is WHS consulting, systems, and regulatory support, mainly for higher risk and complex operations. That work goes well beyond online templates.

However, I regularly get calls from small business owners who genuinely have no idea what they need to meet their WHS obligations. Most are trying to do the right thing but get stuck between generic templates, conflicting advice, and not understanding what the law actually requires of their business.

Because of that, I put together a simple Small Business WHS Compliance Check.

This is not a full WHS system and it is not a shortcut to compliance. It is a starting point to help small businesses understand:
• what WHS applies to them
• where the common gaps are
• why templates alone often do not protect them

Templates can help with basics, but only if you understand the law and how documents must be developed, implemented, and maintained for your specific business. Without that understanding, many businesses still end up non-compliant despite having paperwork in place.

Like many across our industry, I was saddened to read about the tragic workplace incident recently reported at the Tanami mine in the Northern Territory.

My thoughts are with the worker’s family, friends, and workmates, along with the crews and emergency responders impacted by this event. Incidents like this reach far beyond a single workplace and affect entire communities.

With investigations now underway by Police, NT WorkSafe and the Coroner, it is important that the process is allowed to run its course so the full circumstances can be properly understood.

Working in high risk industries over many years, events like this reinforce to me how important strong safety leadership, verification of critical controls, and careful planning and management of lifting and mechanical systems are across all workplaces.

Most importantly, it is a reminder for all of us to continue looking out for one another and to speak up whenever something does not look or feel right.
