The psychosocial risk governance gap almost never announces itself.

It surfaces through a complaint. A turnover spike. A regulator inquiry that arrives with very little warning.

By the time it is visible, the operational risk cost is already in motion β€” disruption, management time, productivity loss, the reputational signal it sends to clients and the workforce.

What is interesting about psychosocial risk governance isn't that organisations get it wrong. It is that most organisations have never had an outside view of whether their governance framework is actually working.

The documentation often exists β€” a risk management policy, a procedure. What those documents cannot tell you is whether the governance framework is performing in the operation.

What does this tell us about how most organisations are managing psychosocial risk?

The question I would ask a CEO is different from the question I would ask a safety manager.

A safety manager needs to know whether safety management systems and the governance framework are compliant.

A CEO needs to know whether governance is protecting business performance, commercial performance and organisational resilience β€” and whether there is genuine executive visibility across contractor relationships, operational risk and psychosocial risk exposure.

Most governance conversations in most organisations never reach the second level. Until a client review, a contract renewal or an investigation makes it unavoidable.

What does that tell us about how governance is positioned in most organisations?

It sits in the safety function. It answers compliance questions. It does not answer commercial questions.

That gap is where my work sits.

The operational assurance review twelve months later told a different story.

Not because the organisation had changed dramatically. Because ISO 45001 certification captures a governance framework at a point in time β€” and operations keep moving. New contractors. Changed processes. Accountability that has shifted since the auditor visited.

What is interesting about ISO 45001 gaps is not that they exist. It is how long they exist before anyone takes an outside look.

Most organisations only find out the gap existed when something forces the question. That is too late.

The contractors had performed well. Three years of solid delivery. No significant incidents.

What the contract renewal review found was something else. The organisation could not demonstrate its contractor governance β€” what oversight looked like, who was accountable, what would happen when performance varied.

The client asked those questions. The answers were not there.

What is interesting about this situation is not the outcome. It is how common it is.

Most organisations find out when it is already too late to prepare.
