Heading home after a hospital stay is a big step, for the person coming home and for the family around them.

Those first weeks can bring new things to manage: medications, wound care, and finding your feet with everyday routines again. The right support at home can make that transition feel a lot more manageable.

Our Registered Nurses, Enrolled Nurses and Support Workers provide after hospital care across Melbourne and surrounding areas, from short-term help after a minor operation or day procedure to longer-term support for more complex recoveries. We support NDIS participants and older Australians through Support at Home.

We're Inclusive Nursing & Support, a Melbourne team of Registered Nurses, Enrolled Nurses and Support Workers providing nursing and support at home across Melbourne and surrounding areas.

We support NDIS participants and older Australians through Support at Home, with nursing care, after hospital care, assistance with daily living, 24-hour home care support, community participation and support for people with psychosocial disability.

Our mission is simple: to support people to hold the power to create opportunity for themselves and others. Every person we work with is different, so we listen first, respect your choices and build support around your goals.

To every carer in our community: we see you, and thank you.

It's National Carers Week and this year's theme is "Care for a carer: Making carers visible". More than 3 million Australians provide unpaid care to a family member or friend, often while juggling work, family and their own wellbeing.

We work alongside carers every day, and we know how much they give. Here are five simple ways you can support a carer this week:

- Check in and ask how they're really going
- Offer a break, even for an hour
- Say thank you
- Lend a hand with a meal, a lift or the shopping
- Encourage them to make time for themselves
