Not every cancellation is avoidable.

But if every cancellation feels justified, your policy isn’t doing what you think it is.
You’re not the bad guy for expecting clients to show up.

You’re the therapist helping them stay in the work.

You might not have heard about how reschedules impact your bottom line.
But it’s actually a major force that affects how you run a sustainable, thriving practice.

Here are 3 reasons why cancellation policies aren’t supported by practitioners:
1. Many practitioners feel guilty enforcing them, fearing it will rupture the therapeutic alliance.

2. Clinicians make the cancellation fee about themselves saying they need paid so pay up. BUT they also worry about charging the fee so this round about feeling of resentment and frustration builds.

3. The industry normalises this wastage as acceptable, rather than applying reflective practice to the journey to understand how both the clinician and practice can support the client to attend. Instead, choosing to say “it’s the client’s journey” or “it’s the clients responsibility”

But my strategies for engagement and the practice are different than most.

Reschedules are a make or break factor in the longevity of a clinicians journey with the practice, minimising or increasing the workload for your admin team and ultimately clinical outcomes for the clients —and addressing them properly ensures your time is valued, your income is stable, and your clients stay committed.

Let’s talk about how to set policies that work for all of the people involved and isn’t just a feeling of a “Money Grab”.

One of my biggest pet peeves in business?

“This is what every practice should be doing.”

Nope. 😅

I’ve spent 20 years working in and around healthcare businesses, and if there’s one thing I’ll keep banging on about, it’s this:

There are very few hard and fast rules in business.

There are caveats. Context. Exceptions. Different clients, teams, service models, goals and owners.

So when someone tells you:

“You need to grow.”

“You need another clinician.”

“You need this KPI.”

“You should be aiming for this number.”

My brain immediately goes:

Okay… but why?

What problem are we actually trying to solve?

Does that advice make sense for this practice?

And will the decision we're making today still make sense five years from now?

I don't want you building your practice from a collection of things somebody on the internet said you should do.

I want you to understand your business well enough to question the advice before you implement it.

Sometimes the smartest business question is simply: “Why are we doing this?”

Bigger team. Bigger revenue. Bigger practice.

Cool… but is that actually what you want? 👀

I’ve never been interested in growth for the sake of growth.

I want to know what you want your business to give you.

More freedom? More profit? More time? A team you actually enjoy leading? Something you can eventually sell?

Because “bigger” and “better” are not automatically the same thing.

Build the business that supports your version of success.

Not the one the internet told you you should want.

The #1 question we get about running a profitable private practice?

💬 “Should I charge a cancellation fee for reschedules?”

And today, we’re answering it once and for all.

Yes - you absolutely should.

Not as a punishment, but as a way to reinforce commitment, protect your time, and ensure clients take their journey seriously. When you set boundaries, you create a practice that values both client outcomes and your own sustainability.

Here’s the reality: Every waived fee is time, income, and energy you’re giving away. But when clients know their session is actually theirs and as a part of that - they pay for the time allocation then they’re far more likely to show up when they book an appointment—and that consistency leads to better outcomes.

Unpopular opinion incoming. 🚨

Waiving cancellation fees too often doesn’t make you kind - it makes your practice unsustainable.

Let me explain…

When you don’t enforce your cancellation policy, you’re sending the message that therapy is optional. Clients start deprioritising their sessions, reschedules increase, and suddenly, your schedule (and income) are a mess. Plus, every open slot is time you could have given to a committed client.

A strong boundary isn’t a punishment - it’s a sign that their progress (and your time) matters.

That’s my take. What do you think? ⬇️

Turns out my definition of success has changed a bit. 😂

I still love business.

I still love strategy.

I still want to make money.

But I also want to take the dogs for a walk in the middle of the day.

I want to travel.

I want time with the people I love.

And I don't want a business that requires me to be available every bloody minute for it to function.

Maybe that's why I'm so obsessed with helping practice owners build businesses that don't rely entirely on them.

Because what's the point of building something “successful” if you can never actually leave it?

Anyway.

The dogs would like it formally noted that midday walks are, in fact, a critical activity. 😂

Yesterday morning I posted about Ahpra's new conflict of interest when practices tie bonus structures to KPI's.

First thing I suggest you do is review your contractor model. Why not your employee model? Because your employee model does not put you in as much risk as the contractor model.

Your contractor model does not clearly define any kind of governance model. It is so much easier to give a contractor KPI's now under this Conflict of Interest Framework than your employees. And honestly, that's where the most of your wastage is happening.

You have a 60/40, 70/30, 80/20 split for your supplier to return low quality services. And that my friend is the largest aspect of wastage, exhaustion and industry inefficiencies than not being able to demand a retention from your employees.

October shouldn't be the first time you're asking:

Who is covered?

How are they currently classified?

What work are they actually doing?

What will their new minimum be?

What does that do to our workforce costs?

Because 1 October isn't planning day.

It's commencement day. 😅
