What makes an investigation effective?

When something goes wrong, it can be easy to focus on what happened and who was involved.

But effective investigations should go further — helping us understand why something happened, what factors influenced the outcome, and what we can learn from it.

Instead of focusing on blame, asking questions such as:

🔹 What was happening at the time?
🔹 What influenced the decisions that were made?
🔹 What can we learn from this?
🔹 What could we change to improve the system?

A learning-focused approach can create greater opportunities for meaningful improvement.
