🧬 CDC Categories A, B and C: What do they really mean?
When biological threats are described as Category A, B or C by the CDC, it is easy to assume that the letters simply rank agents from the most dangerous to the least dangerous.
That is not what the CDC framework was designed to do.
The categories were developed to help public health authorities prioritize preparedness against potential bioterrorism agents. They consider several factors, including potential health impact, ease of dissemination, capacity to cause public fear and disruption, and the level of surveillance, diagnosis and response planning required.
🔴 Category A: Highest preparedness priority
These agents may cause major public health consequences, high mortality or widespread disruption, and require specific preparedness measures. Examples include anthrax, smallpox, plague and botulinum toxin.
🟠 Category B: Second preparedness priority
These agents are generally moderately easy to disseminate and usually cause lower mortality than Category A agents. They still require reinforced surveillance and diagnostic capacity.
🔵 Category C: Emerging threats
This category covers pathogens that could become significant future threats because of their availability, ease of production or dissemination, and potential public health impact.
⚠️ But focusing only on Category A is not enough
A biological attack may not be intended solely to cause fatalities. An adversary may instead seek to incapacitate populations or fighter, overwhelm healthcare services, disrupt essential operations or generate widespread fear. Agents outside Category A may therefore remain operationally significant, depending on the objective, dissemination method and exposure conditions.
👉 Threat Detection Cannot Be Limited to a Category.
