Have you ever read a care note that just says "refused"?
No reason. No detail. Just refused.
It happens all the time, and it tells us very little about the person we're caring for. Did they want a cup of tea first? Were they tired? Did the approach feel rushed?
I've written a blog this week about what good care notes really look like, why "refused" on its own isn't enough, and four simple things every entry should capture instead.

One of the main things we often see during mealtimes is residents being offered several meal choices, but the records don’t reflect the level of support or encouragement staff have provided.

Key details are often missed such as whether fortified foods or supplements were given, or how much support was required.

Action should be taken at the time weight loss is identified, not delayed until the next audit or care plan review. Nutrition care plans are live documents and should reflect current support needs, changes in appetite and any clinical concerns.

Not having consistent oversight of PRN medication can lead to errors and missed opportunities to review what people really need.

If a PRN hasn’t been used for 3 months, is it still required?
If it’s being used regularly, should it remain a PRN?
Does the PRN protocol clearly describe when to use it, including non-verbal signs of pain or distress relevant to the person?

Lack of review or poor documentation in this area can breach Regulation 12: Safe Care and Treatment, specifically Regulation 12(2)(g) — ensuring the proper and safe management of medicines.
