The same patient in their twenties, forties and sixties is not, biologically, the same patient. Hormonal changes across the lifespan mean treatment planning has to evolve too.

Adolescence and early adulthood bring sebum, acne and post-inflammatory hyperpigmentation. Pregnancy and postnatal recovery bring melasma, sensitivity, contraindications for many treatments and delayed recovery. Perimenopause and menopause bring declining oestrogen, laxity, dryness, volume loss and treatments that no longer last as long. Andropause quietly affects men from midlife, changing skin thickness and facial structure. And thyroid or adrenal conditions may present at consultation before diagnosis, allowing thoughtful onward referral.

Patients who understand why their skin is changing are more likely to trust the treatment plan you build around it.

"I'm insured for aesthetic treatments, so I must be covered for this." Not necessarily.

Cover should never be assumed. A practitioner may be qualified to prescribe a medicine, but that doesn't automatically mean every use of it is covered by their policy.

Before adding any new treatment or service, check that the treatment is specifically covered under your policy, that your profession and qualifications meet the insurer's requirements, whether additional training is needed, that the treatment sits within your scope of practice and competence, whether there are restrictions around off-label use, and that the product and supplier meet policy requirements.

If a treatment is not listed on your policy schedule, or you're unsure whether a particular use falls within cover, speak to your insurer before providing it.

"The training is really key."

That's Dr Samantha Hills, Clinical Director at Lynton UK , on this month's Aesthetics Business Cast.

The BMLA sets out three distinct types of training every laser and IPL practitioner should have. Treatment-specific training on how to actually deliver the treatment. System-specific training on how to select the right settings for the particular machine. And laser safety training, also known as the "core of knowledge".

Some manufacturers cover all three. Many will only deliver basic system training and no more. Which matters, because when Sam sees adverse events, it's usually one of those gaps showing up in practice. Incorrect skin type assignment. Overlapping during treatment. Treating patients with recent sun exposure. Small, avoidable mistakes that better training would have prevented.

Back in 1996, we offered a basic medical malpractice policy. Quarter of a million pounds of indemnity. That was it.

Five hormones that shape skin, and quietly shape treatment outcomes:

Oestrogen supports collagen, elastin, hydration and healing. As levels decline, expect dryness, thinning, reduced elasticity and slower recovery.

Progesterone affects sebum, inflammation and the skin barrier. Fluctuations can drive breakouts, sensitivity and hydration changes.

Testosterone is present in all patients. Higher levels drive sebum and acne. Declining levels affect firmness, thickness and collagen support.

Thyroid hormones can present at consultation before diagnosis. Hypothyroidism: dry, coarse skin, puffiness, delayed healing. Hyperthyroidism: sensitivity, flushing, fragility.

Cortisol rises with persistent stress, reducing collagen production, slowing healing and increasing inflammation.
