About the Role
We’re looking for a caring, organised Care Enhancement Coordinator to help improve the experience and outcomes for our clients across Dundee & Angus. You’ll work behind the scenes to enhance care plans, support smooth service delivery, and strengthen connections between clients, families, and our care team. This office-based role also includes the flexibility to support with care visits if needed.
Key Responsibilities
Enhance Client & Family Experience: Be a trusted point of contact for clients and families. Listen to needs, provide reassurance, and ensure their voice is central to care decisions.
Care Plan Enhancement: Review and enhance personal care plans so they reflect individual goals, preferences, and changing needs. Ensure plans are person-centred, up to date, and meet Care Inspectorate + SSSC standards.
Service Enhancement Through Rota Planning: Create and maintain rotas that provide consistent, well-matched care. Communicate changes clearly to reduce disruption for clients and staff.
Stakeholder Engagement: Share clear, timely updates with families, GPs, social work, and partner agencies to promote joined-up, enhanced support.
Team Support & Liaison: Support the care team with daily queries, share key updates, and help resolve issues so staff feel confident delivering high-quality care.
Hands-On Care Support: Occasionally provide direct care support to enhance service continuity, e.g. covering short-notice gaps or supporting complex visits.
What We’re Looking For
Experience in care, health, social work, or a coordination/support role focused on improving outcomes for people.
Excellent communication skills – warm, clear, and confident by phone, email, and in person.
Organised, proactive, and calm under pressure with strong attention to detail.
IT literate – comfortable with care management systems, email, and MS Office.
Commitment to dignity, choice, and person-centred care in line with Care Inspectorate principles.
SVQ Level 2/3 in Health & Social Care or equivalent is desirable.
PVG check required.
