Maternity Cover - starting Mid-May 2024

Adult Social Care Assessor required to undertake assessments of vulnerable adults living in their own homes in the community are of Hyde and Mottram Longdendale. Working in partnership with the individual to capture a Person Centre care plan to meet their care needs, wishes and preferences, with the aim of maximising independence, wellbeing and choice. The job role involves working with other Social Care professionals, such as Adults Social Services, Social Workers, GP’s and District Nurses. Will be required to work Oncall duties on a rotated system.

Temporary Contract: 9-12 months
Monday - Friday 8:30am-17:00pm
