A recent Public Accounts Committee report on NHS England reveals a troubling disconnect: while there’s a pressing need to shift from hospital-based to community care, senior officials appear reluctant to embrace transformative change.
At ACESO, we believe the solution for NHS’s challenges lies not just in moving services into communities – but in fundamentally reimagining how communities participate in their own healthcare journey.

The WHO-recommended Participatory Learning and Action (PLA) approach could transform NHS healthcare delivery by empowering local communities to identify their needs, share vital local knowledge, and actively shape preventive health initiatives.

The report calls for “truly fresh ideas and radical energy” in NHS transformation. PLA could provide exactly this – enabling NHS communities to co-create solutions that strengthen preventive care, optimize resource allocation, and significantly reduce pressure on hospital services. This approach perfectly aligns with NHS’s planned ‘three big shifts’ towards community care, digital transformation, and prevention.

The evidence from our global experience shows: when communities take ownership of their health journey, the results are transformative. We believe the community-first PLA approach could offer the NHS an innovative path to achieve its ambitious goals for healthcare transformation.

The challenge of cervical cancer in 2025 remains significant, with approximately 660,000 new cases globally. What’s particularly concerning is that 94% of the 350,000 deaths occur in low- and middle-income countries - precisely the regions where Aceso has built strong partnerships and implemented successful community health programs.

Drawing from our experience in developing evidence-based assessment tools and implementing community-based health interventions, we understand that effective cervical cancer prevention requires:Culturally appropriate community engagement strategies
,Strong partnerships between healthcare providers and local communities.
Evidence-based implementation of screening programs
Integration with existing maternal health services
Through our collaborations with organizations like the NIHR Collaboration for Leadership in Applied Health Research and Care, we’ve developed frameworks that can be adapted for cervical cancer prevention programs.

This Cervical Cancer Prevention Week, we at Aceso emphasise the importance of community-led health initiatives. Our experience across multiple countries has shown that when communities are actively involved in health program design and implementation, prevention efforts are more successful and sustainable.

The global health challenges are on a scale previously unseen. Today, the WHO took one huge step forward to respond to it, launching an emergency appeal of $1.5 billion for 2025. It’s not about numbers, it’s reaching more than 305 million in need of health support-most of them in war conflict zones like Gaza and Afghanistan.
We at ACESO see firsthand, working in global health, how critical this kind of support is. Our teams on the ground witness daily how access to basic healthcare can transform communities. From helping mothers safely deliver their babies to ensuring children get their essential vaccines, every bit of support creates ripples of positive change.
What the WHO is appealing for is just this: to do what we try to do each and every day, that is to strengthen health systems, fast-tracked responses to outbreaks of diseases, and ensure access to primary essential health services to the most vulnerable communities. About building a world where a person’s place of birth shall no longer determine if one will live or die.

🎉 Congratulations to Darlington David Faijue, Allison Osorio Segui, Kalpita Shringarpure, Ahmed Razavi, Nadeem Hasan, Osman Dar, and Logan Manikam on their new publication! 📄👏
Their systematic review examines governance mechanisms within the One Health framework, integrating human, animal, and environmental health. The review, registered with PROSPERO, analyzed literature from 2000 to 2023 and selected nine studies for in-depth analysis.
Key findings highlight the importance of stakeholder engagement, intersectoral coordination, and sustainable funding. However, challenges like inadequate collaboration and funding constraints hinder progress. The paper calls for stronger governance frameworks to improve health outcomes and sustainability.
