New results from the NHS PROGRESS pharmacogenomics pilot are striking: 91% of patients starting a new medicine in primary care carried a clinically relevant genetic variant, and one in four had their prescription changed as a result.

Clozapine is often the only antipsychotic that works after others have failed. Everyone taking it needs regular white blood cell monitoring, and a low count can end treatment.

ADAN, caused by a variant in the ACKR1 gene, gives people naturally lower white blood cell counts with normal immune function. It is more common in people of African or Middle Eastern ancestry. Standard monitoring thresholds were not calibrated with that in mind.

CERSI-PGx, the UK’s regulatory science centre for pharmacogenomics, has now published guidance covering who to test, when, and what thresholds should apply for Duffy-null patients. Written for the NHS.

Why does this matter? Common remedies including turmeric, ginger and black seed can affect CYP2C9, an enzyme involved in how certain medicines are metabolised.

For patients of South Asian heritage, that figure can rise to more than half.

Genetic variation affects how the drug is metabolised, and it doesn’t follow assumptions. As one clinician puts it: pharmacogenomic testing is colour-blind. It only reads your genes.

A large randomised trial of genotype-guided antidepressant prescribing has published, and it went both ways.

ADOPT PGx randomised 1,460 patients across US primary care and psychiatry. 692 had an actionable CYP2D6 or CYP2C19 phenotype.

At 3 months, no difference against usual care. That was the primary endpoint, and it was missed.

At 6 months, remission was significantly higher in the genotype-guided group. 48.3% versus 39.4%.
