Root Cause Analysis is a critical part of Plan Do Check Act cycles - it acts as the Check and Act stages. When things go wrong, without undertaking an investigation of some kind and implementing actions, events will repeat themselves.

On this course we explore the importance of RCAs, how to undertake them well, and common pitfalls we so often come across along with some solutions.

When RCAs are done well, they are surprisingly effective at reducing problems and give remarkable returns on effort.
