This image sums up a conversation I’ve had more times than I can count.

On paper, everything looks under control.

The system is in place. Training has been delivered. The correct documents exist.

But when you ask one simple question —
“How do you know this is actually working?” — Things get vague.

What usually follows isn’t evidence.
It’s reassurance.

• “We’ve never had a serious incident.”
• “People know what they’re doing.”

Ever walked onto a site and felt like nobody wanted to talk to you? That was me last week.

I walked onto a site recently and you could feel it straight away. Arms crossed. Short answers. Eyes following me like I was there to catch someone out. Safety hat on, clipboard energy — even though I didn’t have a clipboard.

I tried chatting. Polite, but closed.

🔸“No issues.”
🔸“All fine.”
🔸“You’ll have to speak to the supervisor.”

I knew what was really happening. They didn’t see me as someone they could talk to. They saw “safety” walking towards them.

So I did something different. I got back in my car, drove to the café down the road, and came back with bacon butties and coffees. No speech. No agenda. I just put them down and said, “Help yourselves.”

That’s when everything changed. Lads started wandering over. Chat turned into work talk. Work talk turned into real issues. “This job never gets set up properly.” “We’ve raised that three times.” “That machine’s a nightmare, but nothing ever changes.”

None of that came out when I was the safety professional. It came out when I was just a person who’d taken the time to listen. That’s the bit people miss about safety. It’s not the rules that stop people speaking. It’s how safe they feel talking to you.

Once the conversation opened up, we logged the issues straight into Safety PAL there and then. Not later. Not after a meeting. The workers could see what was being raised, who owned it, and what would happen next.

That’s when trust starts to build. Because when people see that speaking up actually leads to change, safety stops being “that annoying thing” and starts feeling like something that’s on their side. You don’t need to be softer. You don’t need to be stricter. You need systems that make it easy to listen — and easy to act — while the conversation is still happening. Yep, two out of four ladder legs on a step, one worker climbing, one holding it. Somehow it “works”… until it doesn’t.

This HSE prosecution is painfully familiar.

A worker fell through an unprotected floor opening and suffered serious injuries. The company has now been fined £60,000.

What’s uncomfortable is how ordinary this was.
- Boards laid across an opening.
- No edge protection.
- No clear ownership.

Nothing dramatic — just a hazard that slowly became normal. This is how people get hurt. Not through recklessness, but through assumptions:
- “Someone’s sorted that.”
- It was fine yesterday.”
- “We’ll deal with it later.”

Safety fails quietly, long before it fails loudly. Temporary risks need to remain visible, owned, and monitored — not just documented once and forgotten.

That’s precisely where systems matter. When hazards, actions, and ownership are live and visible, they don’t fade into the background.
