One of the most common questions we get asked is about care plans, what to include, how to write them and where to get a template.
We’ve put together a practical care planning guide on the website covering everything from what a good care plan should include, to how to write it in the person’s own voice, to when and how to review it.
There’s also a free downloadable assessment template in Word format, laid out in the same way as the care planning feature in the app, so it will feel familiar if you’re already using PocketCarer. You can personalise it with your own logo and details, fill it in with a new client and upload it straight into the app.
