By consulting and involving users of the service and, where appropriate, families and carers, in agreeing and implementing individual needs-led, holistic support plans and risk assessments. By promoting multi-disciplinary working with primary, secondary and tertiary health and social care providers. By providing opportunities for people to learn and develop skills through educational and awareness workshops and the facilitation of client-led self help support groups.
