Once you’ve filled it in you can attach it to your GP’s notes, add it to your medical care plan if you have one, or leave it on or near the fridge for paramedics/neighbours/micro-providers to use.

If you know that there is a time period where you need to arrange temporary care for the person you look after, phone 0300 123 2224 to reach the duty team at Somerset Direct and ask for the temporary emergency respite service.
