Did you know you can use a website offline…without internet connectivity? Hahaha.

From March to July of this year, my team at Kosmos Consult developed a national healthcare management platform engineered on offline-first mechanics.

This system, MedLib, is inspired to resolve the critical healthcare challenges contextual to Liberia’s volatile electricity and internet connectivity.

—> Liberia doesn’t have a unified healthcare management system.
—> Even if they build one, they’d need physical servers which the country’s power and internet volatility will challenge.

Now imagine Kofi goes to JFK. He is registered, sees a doctor, gets his laboratory investigations done, receives his prescription and goes home.

Then a few months later, Kofi travels to Nimba and becomes sick again. He visits another hospital, and the first thing the doctor asks him is his medical history.

Kofi starts explaining but he cannot remember all of the details. And the doctor does not know what medication he previously received, what laboratory results were obtained or what happened during his previous admission.

This is happening because Kofi’s healthcare history is stored in JFK, which is in Montserrado and far from Nimba.

This fragmentation is resolved with MedLib. Instead of building another hospital management software where every hospital has its own isolated system, we built a unified healthcare architecture where hospitals, clinics, laboratories, pharmacies and other points of care can operate within one connected national ecosystem.

MedLib is built on a Master Patient Index, which is the foundation for a longitudinal patient record. Every subsequent encounter, laboratory result and prescription can be linked to that patient’s global identity, while access to the information remains governed by authorization and consent.

So when Kofi visits another hospital, the system does not have to ask, “Who is this patient?” and start his story all over again.

It already knows. This prevents Kofi from retelling his medical history at every hospital he visits.

But there is another problem.

How do you build a national digital healthcare system for Liberia when the power and internet are unstable? What happens when the power and internet go off in Liberia?

Does the hospital stop registering patients?
Does the pharmacist stop dispensing medicine?
Does the laboratory stop recording results?
Does the clinician lose access to the information already available on the device?

We did not want the answer to be yes.

So MedLib was engineered around an offline-first architecture.

This means the system does not treat internet connectivity as a permanent requirement for every operation. When the network is available, information synchronizes with the central system. When the network disappears, the local environment can continue working with cached information and queued changes, which are synchronized when connectivity returns. The architecture is specifically designed around graceful degradation rather than crashing when the network becomes unavailable.

And that’s what I asked initially, and again, I affirm. Yes. You can use a website offline.

The technology of MedLib is sophisticated.

MedLib has to know what information is available locally, what information has already been synchronized, what information was created while offline and what needs to be sent back to the central system once connectivity returns.

It also has to reconcile conflicts so that two different facilities do not accidentally overwrite one another’s information when the network comes back.

But there is another problem that MedLib addresses: Inventory.

Let’s say Kofi has malaria and visits JFK.
The doctor diagnoses him and prescribes Artemether and Lumefantrine 480/80mg.

Kofi walks to the pharmacy. The pharmacist checks the shelf but the medicine is finished.

Now everything that happened before that point becomes almost useless to Kofi.

The doctor did his/her job.
The laboratory may have done its job.
The prescription was correct.

But the medicine is not there.

This is why we paid particular attention to inventory tracking in MedLib.

The system is designed so that inventory does not live separately from what is happening clinically.

When a medicine is prescribed and dispensed, the pharmacy inventory can reflect that transaction. The system can then monitor consumption, estimate the remaining stock runway and generate an alert when the available quantity approaches a critical threshold.

So instead of the hospital discovering that a medicine is finished after the last patient arrives, the system can tell them that they are approaching that point.

That is a very different model of inventory management. It moves the hospital from reacting to shortages to anticipating them.

And this is not limited to counting boxes.

MedLib’s inventory architecture considers historical consumption and delivery lead times when determining replenishment thresholds. The operational standard sets a target of reducing stockouts of important supplies, including antimalarials and maternal health supplies, by 30% to 40%.

If several hospitals and clinics are consuming a particular medicine faster than usual, that information does not have to remain hidden inside their individual pharmacies. The system can begin giving the health network visibility into consumption.

You can know what is available.
You can know what is being consumed.
You can know where shortages are developing.

And eventually, you can make procurement decisions based on what is actually happening rather than waiting for someone to call and say, “We have run out.”

The same architecture supports the other parts of healthcare too.

Clinical care.
Laboratory.
Emergency triage.
Appointments.
Beds.
Billing.
Insurance.
Telemedicine.
Public health.
Workforce management.
Security.
Audit.

If something unusual begins happening across several facilities, public health authorities would see the pattern.

And because MedLib is unified at the national level, the Ministry of Health does not only get to see what is happening inside individual hospitals. It can also begin to see what is happening across the country.

This is where national health surveillance comes in. Imagine several hospitals in different parts of Liberia beginning to report an unusual increase in patients presenting with similar symptoms. Individually, each hospital may see a few more patients than usual.

But when those signals are connected across the healthcare network, it becomes surveillance.

MedLib is designed to give the Ministry of Health that visibility through national surveillance, syndromic monitoring, outbreak alerts and geospatial health intelligence.

Instead of waiting for an outbreak to become severe and then trying to understand where it started, the Ministry can begin seeing the pattern while it is still developing.

That is what makes MedLib a need for Liberian Healthcare.

We are seeking to submit MedLib to the Ministry of Health of Liberia hopefully right now or as soon as possible.

Not as an idea on paper.

But as an architecture and platform that has already been designed around the country’s healthcare and infrastructure context, with operational standards documenting how the system is intended to function.

We also want to make MedLib available for bidding.

If the Ministry of Health wants to evaluate it, audit it, challenge its architecture, modify it or integrate it with existing national systems, we are open to that.

If another qualified organization or consortium wants to bid for its further development, deployment or national implementation, that conversation is also open.

Because our objective is not to say that we built MedLib.

Our objective is to see how much impact MedLib can bring to Liberia.
