The answer for all all those who are asking me …… why I have not shifted to robotic phaco???

Robotic (femtosecond laser–assisted) phacoemulsification
1. High cost
• Very expensive machine, maintenance, disposables
• Increases per-case cost → difficult to justify
2. No proven superiority in outcomes
• Visual acuity, complication rates, and long-term results are similar to good manual phaco in most studies
• Clinical benefit over an experienced surgeon is marginal
3. Learning curve
• Docking, centration, laser parameters need experience
• Initial cases may take longer
4. Increased surgical time
• Additional steps: docking, laser room transfer (if not integrated)
• Overall OT turnover may reduce
5. Docking-related issues
• Suction loss
• Subconjunctival hemorrhage
• Corneal folds affecting capsulotomy accuracy
6. Incomplete or irregular capsulotomy
• Tags or micro-adhesions → risk of capsular tear if not recognized
7. Laser-related complications
• Capsular block syndrome
• Pupillary constriction (prostaglandin release)
• Transient IOP spikes
8. Limited usefulness in certain cases
• Small pupil
• Dense corneal opacity
• Poor fixation / uncooperative patient
• Advanced white or hypermature cataracts
9. Space and logistics
• Requires additional room space and infrastructure
• Workflow becomes more complex
10. Dependence on technology

• Machine downtime can disrupt surgical schedules
• Surgeon may lose manual adaptability if over-dependent

Bottom line:
Robotic (femto-assisted) phaco offers precision and marketing appeal, but cost, logistics, and lack of clear outcome superiority remain major disadvantages—especially where high-volume, cost-effective cataract surgery is needed.
