A recent incident at a railway station highlighted not just the entrapment—but the way the rescue was carried out.

To evacuate passengers, the entire landing door was forcibly removed, causing damage to equipment and creating additional safety risks.

What Went Wrong

• Rescue handled by untrained personnel
• Use of improper methods
• Damage to critical safety components
• Increased risk during rescue itself

Why This Is Dangerous

• Landing doors are safety components, not access panels
• Improper rescue can lead to falls, entrapment, or further injuries

The recent harassment news at a Nashik office reminds me of my wonderful experience with an American company where ethics - values were taken very seriously.

Values like respect, integrity, and the freedom to speak up without fear were strongly followed. Systems like an Ombudsman without disclosing the name of the victim and no-retaliation policy protected everyone—employees, vendors, and even subcontractors.

POSH training is mandatory, but companies need to go beyond just compliance.

Regular communication from leadership, awareness via
posters, training, facilitating Ethics champs in town hall, top agenda in monthly reviews and yearly online mandatory courses are important to build a safe workplace culture.

It’s time we follow ethics not just on paper, but in real practice.

Elevator; Uncontrolled Upward Movement During Maintenance – A Critical Wake-Up Call for all stakeholders holders-

A recent incident in China reported an empty elevator car suddenly moving upward and slamming into the shaft ceiling during maintenance.

No casualties
But a major safety lapse exposed

Possible Cause
Free-wheeling of an empty car may indicate:
• Brake failure or loss of holding torque
• Inadequate control during maintenance

Why This Is Dangerous
• Uncontrolled car movement
• High risk to maintenance staff & passengers
• Failure of a critical safety system
- High cost for repairs
- A long down time
- Damages the building structure

How It Could Be Prevented

ACOP (Ascending Car Overspeed Protection) (as per IS 17900)

✔ Detects unintended upward movement
✔ Activates safety gear automatically
✔ Brings the elevator to a safe stop

Key Message

Are we ready for such indept audits and discipline?
– A Lesson Worth Sharing

Recently, I interacted with a senior ex field engineer who shared an audit experience at an embassy building conducted by an American, third party expert who flew in for just 3 days.

What stood out was not just the depth of inspection—but the discipline to standards.

✔ Every component was checked thoroughly
✔ Critical tests were performed (Brake test at 120% load, buffer impact validation)
✔ Zero tolerance for deviation

The turning point?
The elevator was immediately shut down because the emergency light was not functioning.

Despite suggestions to repair the PCB at site, the auditor refused any temporary fix and insisted on complete replacement—no risk, no compromise. This was verified again by this auditor

The lift remained out of service for 3 days.

Why this matters:
• Safety is not about availability—it’s about reliability
• SOPs are meaningful only when strictly followed
• Temporary fixes can become permanent risks

In many cases, we tend to “manage” issues.
But true professionalism lies in eliminating risk, not accepting it.

3 Kids trapped inside a elevator in Thane , Mumbai – once again a wake up call

TOI reported today of three children trapped in a lift due to a technical malfunction . Though ended safely thanks to timely rescue with the help of firemen, however , it highlights again serious concern.

Regular maintenance is non-negotiable
Emergency systems like ARD must be functional and tested periodically ( helps during power failure)
Trained response is critical—never attempt unsafe rescue. Few members should be trained and authorised for such rescue ( but not using brake releaser)
Awareness among residents, especially children, is essential. Kids should be always be accompanied by seniors
Inspections of elevators by authorities or third party
The unlocking key devices operating properly

Even a short entrapment can turn critical without proper systems in place. In 2025, in a similar incident the father lost his kid trapped inside during rescue. A teen lost his life trapped inside a lift in a public place

Elevator Passenger Rescue – Are We Creating More Risk Than Safety?

Recent tragic incidents of passenger fatalities highlight a serious concern in improper rescue attempts during lift entrapments.

In many cases, untrained individuals—often building staff( lift man) who observe elevator maintenance activities—try to operate brake release devices or attempt rescues without proper knowledge.

This can turn a safe situation into a fatal one.

🔴 Critical Safety Guidelines

❌ Do NOT allow unauthorized persons to operate brake release devices
❌ Avoid makeshift rescue methods like stools or portable ladders
❌ Do not attempt rescue without proper training and control
- ☢️Services provider should not bypass electrically while rescuing.

Key Message

A wrong rescue attempt is more dangerous than waiting.

The safest rescue is always by a trained technician with proper tools and procedures.

Focus is on two-wheeler riders entering villages
Awareness is being created through banners, village leaders, and door-to-door campaigns

Passenger trapped once again !!!

A lift malfunction at Puzhuthivakkam MRTS station, Chennai left 17 passengers, including 4 children, trapped for nearly 2 hours.
• The incident occurred shortly after the recent inauguration of the MRTS stretch
• Passengers were stuck inside the elevator on Sunday night
• Rescue teams eventually evacuated all safely

Key Concerns:
• Long delay in rescue (2 hours)
• Overcrowding inside the lift
• Questions on maintenance and system reliability in a newly opened facility

Takeaway:
Even new installations require thorough testing, proper maintenance, and quick emergency response systems to ensure passenger safety. A third party inspections are always recommended

Serious Lift Accident Reported in Jaipur – A Wake-Up Call

As reported by Dainik Bhaskar (25 March 2026), a lift in a residential society at Jagatpura, Jaipur fell from the second floor to the basement, injuring 8 occupants, including children. One person suffered a fracture.

What is more concerning is that maintenance-related complaints were reportedly raised earlier but not acted upon.

Though the full report is not available we at EIQS has listed few likely causes

❌ No overload system provided

Critical Learnings

• Never ignore repeated breakdowns or complaints

Decided to leave my car behind today and use the Mumbai Metro for a client meeting.

The ride was a dream—covered the distance in just 20 minutes. Efficient, comfortable, and exactly what urban transport should feel like.

But the last mile told a different story.

Despite over a dozen autos at the station, not one agreed to go to Vile Parle East. Ended up waiting another 20 minutes.

Makes you wonder…
We are solving speed, but are we solving connectivity?
