Three reasons stays stuck in prototyping:
1 - No part selection strategy. Trying to print anything instead of identifying what actually makes sense for AM.
2 - No DfAM ownership. If nobody owns design for additive, AM becomes just another way to print the same bad geometry.
3 - No industrial workflow. Without quoting, traceability, QA and repeatability, AM does not scale.
