What happens to muscle oxygenation as exercise intensity increases?

Near-infrared spectroscopy (NIRS) allows researchers to monitor local changes in muscle oxygenation during exercise, providing insight into the balance between oxygen supply and demand. In this introduction to muscle NIRS, we cover the fundamentals behind the measurement and show what these signals can tell us in practice:

🟡 How NIRS measures changes in oxyhemoglobin, deoxyhemoglobin and total hemoglobin.
🔵 How Tissue Saturation Index (TSI) provides an absolute measure of tissue oxygen saturation.
🟡 What happens to muscle oxygenation as exercise intensity increases, including an example from a multistage fitness test.
🔵 How measurements such as arterial and venous occlusions can be used to assess parameters including oxygen consumption, blood flow and reoxygenation.

How do young children’s brains coordinate with a parent during play?
In their exciting new fNIRS hyperscanning study, Terry et al. used two wireless Brite MKII systems to simultaneously measure brain activity in 20 mother–toddler dyads. Children aged 24–36 months built Duplo structures either collaboratively with their mother or individually, separated by a curtain.
The authors found different patterns of mother–child neural coherence across the two conditions. During collaborative play, coherence was greater between the children’s left prefrontal cortex and the mothers’ right temporoparietal junction. By enabling two people to be measured at the same time during a natural, child-friendly activity, the Brite MKII helps researchers explore how early social interaction and collaboration develop.

Can VR-based therapy help us better understand pain recovery after injury?
In a recent pilot study, Mace et al. examined an 8-week home-based therapeutic VR intervention for subacute musculoskeletal pain, using the OctaMon to measure prefrontal cortex activity with fNIRS.

The results showed:
🟡 High feasibility, with 93% valid fNIRS recordings
🔵 No reported safety concerns
🟡 Improvements in pain intensity, pain interference, pain anxiety, pain catastrophizing, mindfulness, coping, and pain self-efficacy
🔵 Changes in medial and lateral prefrontal cortex activity during VR relaxation and distraction tasks

These findings suggest that combining VR therapy with fNIRS may help researchers better understand the neural mechanisms behind nonpharmacological pain management.

Can VR make exercise more engaging without changing its cognitive benefits?
In a pilot study, Kelsey et al. compared traditional cycling with VR-based cycling in adults with overweight/obesity, a group at elevated risk of cognitive decline. Participants completed two 16-minute moderate-intensity cycling sessions, followed by a Stroop task while prefrontal cortex oxygenation was measured with fNIRS and processed in OxySoft.

The results showed similar Stroop accuracy and no significant differences in prefrontal O2Hb between conditions. Response time was faster after traditional cycling in two Stroop rounds, but overall, both exercise modes appeared feasible and produced largely comparable post-exercise cognitive outcomes.
A promising step for VR-based exercise research, and a clear example of how fNIRS can help assess brain responses in more engaging, movement-friendly settings.

How well can virtual reality capture brain activity during real-world behaviors?
A recent study by Dina et al. combined a CAVE virtual reality environment with mobile to measure response inhibition in children aged 3 to 7. During the task, participants interacted with virtual bubbles while the authors used two Brite devices to record cortical activity.

The study demonstrated that this multimodal CAVE-fNIRS setup was feasible and well tolerated in young children, opening new possibilities for studying in engaging, movement-friendly environments.
