IASTM = A fancy acronym to describe rubbing an object on people’s skin that isn’t your hands in order to stimulate dermal receptors that may alter afferent information in the short term and generate a meaning response for the patient.

You can do this:

As fast or as slow as the patient likes

As soft or as hard as the patient likes

In whatever direction the patient likes

With whatever edge of the tool that the patient likes

On anyone you would do manual therapy with your hands on.

You DO NOT need someone to teach you how to do this.... You DO NOT need an expensive class to do this…You DO NOT need a special certification to do this…PLEASE stop paying people to teach you their flavor of tool rubbing. This IS NOT where doctorally educated clinicians should be spending their precious dollars!!

I’m sorry, but pain and injuries are almost never a simple black-and-white scenario.

The vast majority of musculoskeletal pain issues have a plethora of contributing factors and typically manifest due to all of those factors working together to push your body out of its zone of homeostasis.

Those different factors can include but are not limited to:
1. Your current activity levels
2. Your historical activity levels
3. Your current metabolic health
4. Your current immune system function
5. Psychosocial factors such as stress, anxiety, depression, fear, self efficacy, history of trauma, etc.
6. The quality of your sleep
7. Neuroplastic changes in your peripheral and central nervous system
8. Adaptive/maladaptive movement patterns
9. Your recovery strategies and stress management activities
10. Your genetic capacity for adaptation and activity tolerance

If you see somebody saying there’s a simple answer or you reach out to someone for advice and they give you a simple explanation that doesn’t depend On some of the factors listed above DONT TRUST THEM!

KNEE VALGUS NOT AS IMPORTANT AS WE THOUGHT FOR PREDICTING ACL INJURIES?

Great breakdown on this interesting study by

This just published study followed 880 elite female handball and soccer players after performing a 2D movement analysis looking at frontal plane deviations during the single leg squat and vertical drop jump, common tests used to assess ACL injury risk. 722 non-injured players along with 56 females with a history of a non-contact ACL injury were included in this prospective study. 74 females sustained an ACL injury post-assessment in the years following their inclusion into the study with neither assessment correlating in either group to risk of tearing the ACL.

The Takeaway? We don't know what predisposes individuals to higher ACL tears and this study lends credence to not putting too much weight on these two screening assessments when determining ACL tear risk.

Never forget that you are ALWAYS a part of the placebo effect (meaning response) and that every single intervention you perform and interaction you have carries meaning that influences what your patient experiences.

We are completely unable to separate ourselves from this phenomenon in the clinic and it is one of the key factors as to why we can’t blindly believe what we think we see in our clinical results and how we rate the effectiveness of our interventions.

My favorite example demonstrating the non specific effects we impart into treatment is ā€œThe open hidden paradigmā€. Read about what it is and what it means if you haven’t before!!
