- This application is for currently enrolled 𝗖𝗢𝗟𝗟𝗘𝗚𝗘 𝗦𝗧𝗨𝗗𝗘𝗡𝗧𝗦 from the
𝟳𝗧𝗛 𝗗𝗜𝗦𝗧𝗥𝗜𝗖𝗧 𝗢𝗙 𝗖𝗘𝗕𝗨 𝗢𝗡𝗟𝗬 who want to avail of the CASH ASSISTANCE through the Office of Congressman Peter John D. Calderon.
- 𝗢𝗻𝗹𝘆 𝘁𝗵𝗼𝘀𝗲 𝘀𝘁𝘂𝗱𝗲𝗻𝘁𝘀 𝘄𝗵𝗼 𝗵𝗮𝘃𝗲 𝗡𝗢𝗧 𝗿𝗲𝗰𝗲𝗶𝘃𝗲𝗱 𝗮𝗻 𝗘𝗗𝗨𝗖𝗔𝗧𝗜𝗢𝗡𝗔𝗟 𝗔𝗦𝗦𝗜𝗦𝗧𝗔𝗡𝗖𝗘 𝗳𝗿𝗼𝗺 𝘁𝗵𝗲 𝗗𝗦𝗪𝗗 𝗹𝗮𝘀𝘁 (𝗝𝗨𝗟𝗬 - 𝗢𝗖𝗧𝗢𝗕𝗘𝗥 𝟮𝟬𝟮𝟮) 𝗺𝗮𝘆 𝗾𝘂𝗮𝗹𝗶𝗳𝘆 𝗶𝗻 𝘁𝗵𝗶𝘀 𝗽𝗿𝗼𝗴𝗿𝗮𝗺.
- During the payout make sure you have the following requirements (𝟭 𝗼𝗿𝗶𝗴𝗶𝗻𝗮𝗹 𝗮𝗻𝗱 𝟮 𝗰𝗼𝗽𝗶𝗲𝘀):
1. VALID SCHOOL ID or VOTER'S CERTIFICATE or POLICE CLEARANCE; and
2. Certificate of Enrollment (COE) or Study Load with name and signature of registrar.
- 𝗗𝗲𝗮𝗱𝗹𝗶𝗻𝗲 𝗳𝗼𝗿 𝘁𝗵𝗲 𝗮𝗽𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗶𝘀 𝗼𝗻 𝗢𝗰𝘁𝗼𝗯𝗲𝗿 𝟮𝟳, 𝟮𝟬𝟮𝟮 (𝗧𝗵𝘂𝗿𝘀𝗱𝗮𝘆). 𝗡𝗢 𝗘𝗫𝗧𝗘𝗡𝗦𝗜𝗢𝗡.
- Kindly fill-up short answers in 𝗖𝗔𝗣𝗜𝗧𝗔𝗟 𝗟𝗘𝗧𝗧𝗘𝗥𝗦 and 𝗗𝗢𝗨𝗕𝗟𝗘-𝗖𝗛𝗘𝗖𝗞 𝗦𝗣𝗘𝗟𝗟𝗜𝗡𝗚𝗦 (ESP. NAME SPELLING) before submitting your form.
