• Marriage Certificate (if married)
• Diploma/Transcript of Records
• Certificate of Good Moral Character/Barangay Clearance
• Certificate of Employment
• Medical Certificate
• Accident Insurance
• 2 pcs Passport size ID Picture* and 2 pcs 1 x 1 ID Picture*
*ID pics must be of good quality, with collared top and plain
white background
