ANNOUNCEMENT:

To those who renewed their PRC IDs during the Mobile PRC last April 23 & 24, 2015 at the Oroquieta City Gym, you may now claim your new PRC ID at the General Services Office (GSO) located at the first floor of the Oroquieta City Hall and look for Ms. Dinna R. Bulawan. The PRC IDs will be available at the GSO for ONE MONTH ONLY before we return them to PRC Cagayan.

Please bring the following:

• Original claim stub
• Valid ID

If claimed by a representative:

• Authorization letter signed by the registered Professional
• Original claim stub
• Photocopy of valid ID of the registered Professional
• Original and photocopy of valid ID of representative

Professional Regulatory Commission (PRC) Mobile Services on May 10-11, 2014 at the Convention Hall of the Oroquieta Town Center. Services offered: Renewal of Professional ID and Application of Exams.

APPLICATION FOR LICENSURE EXAMINATION:
Documentary Requirements
1. Original and photocopies of Transcript of Records with Special Order and Date of Graduation, with scanned pictures and with remarks “FOR BOARD EXAMINATION PURPOSES ONLY”. Graduates of government schools and institutions/programs accredited by recognized accredited agencies under the FAAP are exempted from SO. Graduates of New Schools/Degree Programs must submit School Recognition and/or Permit to Operate.
2. Original and photocopy of NSO-issued Birth Certificate (if NSO copy is not clear, bring copy from the Local Civil Registrar)
3. For married females, original and photocopies of NSO-issued Marriage Contract (if NSO copy is not clear, bring copy from Local Civil Registrar)
4. Current Community Tax Certificate (Cedula)
5. Other specific requirements as required by the Commission or the Professional Regulatory Board.

RENEWAL OF PROFESSIONAL ID:
Documentary Requirements
1. Professional or duly authorized representative must secure and accomplish a renewal form
2. Professional pays to cashier the renewal fee upon securing his/her printed renewal application.

NOTE:

PRC will provide the official photographer (for a fee) for the picture requirement

*PRC-registered professional acting as representatives may transact with PRC upon presentation of Authorization Letter and his/her professional ID.

**Representatives filing the application and claiming the professional ID in behalf of the professional must present Special Power of Attorney and valid identification of the professional and the representative.

No changes in the fees!

Php 450.00 is the regular fee for Renewal. Fees are subject to the assessment of PRC.

For Change of Status, please bring SECPA of Marriage Certificate and Birth Certificate plus Affidavit stating the reason of the request for change.

Clients may already fill-up ONLINE the Application Form and PRINT it. Or they may opt to have it onsite.

For unclaimed ID’s: We are giving them until Wednesday (May 7, 2014) to notify us so we can give the list to PRC Cagayan de Oro.
