e. Itinerary / Schedule
f. RPHI Entry-Exit Points

Request for Phil. Landing Permit should contain the following info;
a. Purpose of Flight

and copies of the following documents;
a. Aircraft Certificate of Registration
b. Aircraft Certificate of Airworthiness
c. Operating Crews´

License and Medical Certificate
d. Charter Contract - for passenger or cargo tered flights
e. Patient´s Profile - for Ambulance / MEDEVAC Flights
